1.1 Who to Notify
This Section contains 2 sheets on which you can advise who should be notified in case of your incapacitation.
1.2 Copy of My Power of Attorney (POA) – Personal Care
In this section you would add a copy of your most recent power of attorney for personal care.
1.3 Copy of My Power of Attorney (POA) – Financial Resources
In this section you would add a most recent copy of your power of attorney for Financial Resources.
1.4 Copy of Resuscitation/Extra Ordinary Efforts Request
On this sheet, or an inserted sheet, you may outline your wishes when it comes to what lengths should be taken to keep you alive and well.
1.5 Current Arrangements/Wishes
This sheet or an inserted sheet might outline any pre arrangements such as retirement home(s)/ nursing home(s) that you would like to stay at. Outline people you would like to have stay with you if you are to stay in your own home.2.1 To Whom it May Concern
2.1 To Whom it May Concern
This section would normally be a short note stating who to contact immediately. You may also state your organ donation wishes, if it is not possible to find your driver’s license. In addition this sheet can be used for any general instructions such as pets, plants and so on.
2.2 Initial Contacts
This section again is where you put the people that you want notified of your death. One idea is to have a few call captains who can be called and they would then notify others of your death.
2.3 Initial Burial Arrangements
In this section you can insert sheets if you have already made burial arrangements so those around you know who to contact. If you have not pre paid or made burial arrangements it would be good to put any wishes or comments about how you would like to be buried or cremated.
2.4 Obituary & Public Notice(s)
In this section you could write a draft obituary and/ or public notification as well as what news paper/ publications you would like to be published in. You could also itemize some of the highlights in your life which would make the job of those writing your obituary easier and also might assist in your eulogy /homily.
2.5 Copy of Last Will and Testament
In this section you could leave either the original will and testament as long as your lawyer has a copy, or if the lawyer has the original, insert a copy here and provide the legal office information for your executors to retrieve the original if needed.
2.6 Copy of Codicil (if any)
This section could contain your most recent and up to date codicil if one has been created.
2.7 Copy of Personal Items Distributions Memorandum
In this section you could insert a document which would itemize specific personal items that you wish to be left to certain individuals. This way the executors may have clear directions of what has been generically stated in your will.
2.8 List of Insurance Policies
In this section you could list your vehicle, home, and life insurance policies so that your executors could quickly identify and advise your insurers of your death, and could expedite any payments to your estate or beneficiaries. (If beneficiaries have been pre designated in your insurance policies)
2.9 List of Bank/Investment Accounts
This section would contain a list of all bank accounts and investment institutions. Again this will assist your executors to quickly consolidate your financial assets into an estate account.
2.10 List of Wealth Managers/ Advisors
In this section you would provide your list of wealth management companies and account numbers so that your executors can expedite consolidation of your accounts.
2.11 List of Lawyers, Accountant Contacts
This section would contain a list of who your legal advisors might be as well as any other official names of accountants that you might recommend, who are familiar with your legal and financial affairs.
2.12 Copy of Previous Tax Return/ Notice of Assessment
In this section it would be beneficial to provide a copy of your most recently filed tax return and if possible a copy of the most recent notice of assessment. This will give the accountants and executors an idea of any tax liabilities as well as contact information including your social insurance number.
2.13 Location of Legal/ Financial Documents
In this section you could either on the existing sheet or additional sheets add where you keep your current bills whether online or hard copy. Any filing cabinet or safety deposit box where any other document (passports, birth, marriage certificates) might be kept.
2.14 List of Known Assets
In this section you would list any assets such as real-estate, homes, cottages, vehicles, boats, jewellery etc. This will assist the executors and accountants in assigning fair market value. It might be helpful to provide the purchase price so that capital gains or losses can be taken into consideration.
2.15 List of Known Liabilities
In this section you would itemize all outstanding debts such as a mortgage, car loan, bank line of credit or student loans. You might also want to include monthly re occurring payables and supplier names such as your cable, telephone, gas hydro account provider.
